Sylvania Schools has partnered with Toledo Lucas County Health Department to provide a vaccination clinic for students who are 12-15 years of age along with their vaccine-eligible family members.

Second Dose

TLCHD has confirmed that it will be the Pfizer vaccine. You will make the second dose appointment on-site at the time of the first dose for Wednesday, June 9. Please see the Emergency Use Authorization Fact Sheet for the Pfizer vaccine (link).


Students who are 12-17 

Students who are under 18 years old will need to have a consent form signed by a parent/guardian. A parent/guardian can accompany the minor to the appointment, but it is not necessary. If a parent/guardian will not be with the student at the appointment, then the student must bring the consent form signed by the parent. Consent form (link)
